Patient Forms

Our office utilizes a program called Flex to coordinate digital confirmations and appointment reminders for our patients.


One week before your appointment, you will receive a text or email reminding you of your appointment date and time. Approximately 4 days prior to your scheduled appointment, you will receive a text or email confirmation request. You can simply reply to the text or follow the instructions in the email message to confirm your appointment with us in just a few seconds. As always, if you have any questions or would like additional information regarding your upcoming appointment, you may call or text us to speak directly with a member of our front desk team.


We have also recently converted to paperless forms for our check-in process. As much as possible, we will try to send these forms to you by email or text in advance of your appointment. Alternately, new patients (or those who have not been into the office for an extended period of time) may click the "Forms" link above to complete your paperwork in a secure online portal before coming to the office. This will upload directly into your dental chart, creating a more efficient workflow and smoother check-in process upon your arrival.

Our COVID-19 Policy

Due to COVID-19 and the many changes placed upon dentistry by our government and regulatory agencies, you will notice some changes as you present to our office for treatment during the ongoing pandemic.
