Patient Forms and Information
Our office utilizes a program called Flex to coordinate digital confirmations and appointment reminders for our patients. One week before your appointment, you will receive a text or email reminding you of your appointment date and time. Approximately 4 days prior to your scheduled appointment, you will receive a text or email confirmation request. You can simply reply to the text or follow the instructions in the email message to confirm your appointment with us in just a few seconds. As always, if you have any questions or would like additional information regarding your upcoming appointment, you may call or text us to speak directly with a member of our front desk team. We have also recently coverted to paperless forms for our check-in process. As much as possible, we will try to send these forms to you by email or text in advance of your appointment. Alternately, new patients (or those who have not been into the office for an extended period of time) may click the "new patients" link below to complete your paperwork in an online secure portal before coming to the office. This will input directly into your dental chart, creating a more efficient workflow and smoother check in process upon your arrival.

Our office is in-network with many private or employer-based dental insurance plans. To see if we are in-network with your specific plan or policy, please call our office so that we can assist you personally. We may ask you to text or email a copy of your insurance card for our reference. There are hundreds of different dental insurance plans and policies available, and your benefits may vary greatly from one year to the next based on the plan options chosen by you or your employer. To the best of our ability, we will try to verify your insurance information prior to your appointment. However, as insurance plan details vary widely, we encourage you to speak directly with your insurance carrier or your employer's HR representative regarding any specific questions you may have.
At this time, Park Rapids Family Dental is not accepting new patients with state-based insurances such as PrimeWest or Medical Assistance.
Our office accepts cash, check, and most major credit cards. Payment is expected on the day your services are performed, and all patients are responsible for any portion of their balance not covered by insurance. At this time, we are not enrolled to accept CareCredit.
